Casa Esperanza’s Family Care Team, interns and volunteers work diligently to ensure that our families have the resources and support to focus on their healing journey. The mission of Casa Esperanza’s Family Care Team Program is to facilitate access to care, improve quality of life, and reduce the burden of illness for patients of all ages, their families, and their communities.

Family Care Team

Casa Esperanza understands that traveling for medical treatment may cause additional emotional and financial strain on families. To help alleviate these burdens, Casa Esperanza’s Family Care Team works with our families to provide the following resources:

• Coordinate community dinners at Casa Esperanza

• Provide family food boxes

• Provide transportation assistance such as taxi vouchers and shuttle services to UNM hospital and clinics

• Facilitate lodging assistance

• Facilitates access to healthcare, social services, and disseminate information

• Encourages informed decision making, self-efficacy, and disease self-management skills

• Improve resident quality of life by reducing barriers to care, and alleviating financial burdens

• Provide a supportive, culturally sensitive environment based on shared experiences and connection

• Enhance access to our services through our bilingual program staff who determine each family’s need through individual assessments and information collected during initial check-in

• Identifies and addresses implicit bias that could hinder patient-provider interactions and communication
